"Access is denied" error with Indexing Service turned off

We have a customer that is getting an "Access is denied" (to a specific dll in the application's bin directory) error on XP when trying to run an ASP.NET application. All the posts and KB articles I have read (such as http://support.microsoft.com/default.aspx?scid=kb;en-us;329065) say that this happens when indexing service is running. But in this case we have verified that it is not running. I have also looked at file permissions and do not see a problem.

I have run the tlist.exe utility to see if some other application has the dll locked but the answer is no. Interestingly, if I refresh the browser several times, finally the application will run correctly without the Access is denied error. So the problem is not consistent. It comes and goes.

I have read things that say this problem can be due to many things, but apart from turning off indexing service I don't know what the other "things" may be. Can somebody tell me how to debug this?
