there's Omondo plugin for eclipse. i think it can do this.

I think Borland Together can:

But I'm not sure on the details of it, and I'm pretty sure it's not
free. Generating UML from source code is also a built-in feature of
CodeGear RAD Studio (used to be Borland C++ Builder). You can get a
trial version of RAD Studio / C++ Builder here:

A poor mans answer is to use doxygen + graphviz with the UML_LOOK tag
in its configuration file set to YES.
