Error: A dynamic link library (DLL) initialization routine failed

I am getting "A dynamic link library (DLL) initialization routine failed" error
in IIS 6.0 on Windows 2003 server.

I have this same code working on WiN-NT.

Does anyone have an idea how to fix this?
